I/We, [Your Full Name(s)], residing at [Your Address], [City, State, Zip Code], hereby notify you of my/our intent to homeschool my/our child(ren), [Child(ren)'s Full Name(s)], born on [Date(s) of Birth], starting from the [Start Date of Homeschooling]. This letter is to comply with the New York State Compulsory Education Law.
As required, I/We will submit an Individualized Home Instruction Plan (IHIP) for each child being homeschooled on or before August 15th, or within four weeks of the commencement of home instruction if the instruction is to begin after the start of the school year.
